Miami-Dade County
Start with Miami-Dade’s Declaration of Domicile instructions. The county requires the application to be notarized before submission and lists a $10 recording fee, plus $3 for a certified copy.
The current location directory lists the County Recorder at 20 NW 1st Avenue, Suite 5.243, Miami. Check the mailing instructions separately.
Miami-Dade accepts mail submissions and asks for a self-addressed stamped envelope. It lists an approximate four-week return time. Personal and company checks are not accepted; mail payments must be a money order or cashier’s check.
Broward County
Use Broward’s Records, Taxes and Treasury Division. The official declaration form is available directly from the county.
Check the recording department’s declaration page for current submission and fee instructions. Confirm the charge for a counter oath separately from the recording charge.
The form is a sworn statement about your home. Complete its questions about current and former residences accurately, including any additional residences it asks you to disclose.
Palm Beach County

Collier County (Naples)
Find the declaration under Collier’s official recording forms.
The Clerk & Comptroller lists 3315 Tamiami Trail East, Suite 102, Naples. Confirm the appropriate recording counter and signature requirements before visiting.
Use the county’s current fee schedule and submission instructions when choosing between in-person, mail, and electronic services. Electronic recording availability does not mean an unsigned form can be uploaded without completing the required oath.
Lee County (Fort Myers and Cape Coral)
Lee’s official declaration instructions and form are especially useful. They list $10 for recording and $10 for an optional in-person notary service.
The listed counters are 2115 Second Street, 2nd Floor, Fort Myers, and 1039 SE 9th Place, 2nd Floor, Cape Coral.
For mail, the instructions give P.O. Box 2278, Fort Myers, FL 33902, addressed to the Official Records Office. Include the requested return envelope and an accepted payment. Do not mail cash or card information.
Lee says one form per household is required. Each person’s statement must still match their circumstances; a shared form does not combine spouses’ tax-residency facts.
Sarasota County

Confirm the current recording address and service availability with the clerk. A form submitted to the correct recording department follows a different process from filing a document in a court case.
If timing matters, ask about current processing rather than assuming a mailed or electronic submission will be recorded the same day.
Orange County (Orlando)
Use the Orange County Comptroller’s Official Records Department. Its forms page links to the Declaration of Domicile PDF.
